Every time this happened to me, it was that rubber grommet. For a long time, I never saw it. When I drove mine daily, after I changed plugs, I would put a little black RTV around it, over time that grommet develops a 'memory' and is shaped to the hole, if you are not putting it back int the right spot then you will have a gap.
