It sounds like the bearing is gone. You will have to replace the tensioner. I had the same thing happen to an idler pulley on my supercharged 98 GT. The bearings are sealed, so there is no other way...

Buy yourself a hand pump and fill it through the fill hole on the tranny. That's how I fill mine. The pumps cost about $10 at any auto parts store. IMO, removing the shifter is unecessary work.

13-14 psi at the track, and 22 psi on the street. The track pressure indicated got me consistent 1.6 short times with 1.5's on two occasions.
